Support at Home (Home Care Services)

We understand and appreciate that many older community members wish to remain living independently in their own homes for as long as possible. With the right support, this can often be achieved safely, respectfully and comfortably. Wenonah Community Care provides government-subsidised services that help older people maintain their independence, wellbeing and connection to their community.

As a highly regarded, locally based rural provider, Wenonah Community Care understands the importance of reliable services, familiar, friendly faces and thoughtful and appropriate support that enables older members of our region to stay at home.

Our array of services support people with everyday activities, personal care, health needs and social engagement, allowing them to continue living safely at home.

Wenonah Community Care currently supports 86 participants through the Support at Home program, in addition to more than 70 participants receiving Commonwealth Home Support Programme (CHSP) services across our local community.

Seeking help at home is a positive and important step when seeking to maintain at home independence. Even small amounts of support can make a meaningful difference to the safety and quality of life that our older generation deserve.

What is the Support at Home Program?

The Australian Government introduced the Support at Home program as part of aged care reforms under the new Aged Care Act, which commenced on 1 November 2025.

The Support at Home program replaces the previous Home Care Packages (HCP) program, creating a simpler and more consistent system for delivering aged care services in the home.

Support at Home is designed for people who require more coordinated or complex care and support to remain living safely, happily and independently at home.

The program aims to make aged care services delivered at home:

  • Easier to access
  • More consistent across Australia
  • More flexible and personalised
  • Clearer in pricing and funding

Services We Provide

Wenonah Community Care offers a range of services designed to support independence, health and wellbeing at home; these include:

  • Clinical Care
  • Health-related services delivered by qualified professionals, such as:
  • Nursing Care
  • Wound Care
  • Medication Management
  • Independence Support

Support with Personal Activities and Maintaining Community Connections, Including:

  • Showering and dressing
  • Mobility Assistance
  • Medication Prompts
  • Transport to Medical and Other Essential Appointments
  • Transport for Shopping or Social Engagements
  • Everyday Living Support

Practical Help Around the Home, Including:

  • Cleaning and Laundry
  • Gardening and Lawn Care
  • Meal Preparation

Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme (CHSP) provides base-level support for older people who need only a small amount of assistance to remain living independently at home.

Through CHSP, Wenonah Community Care provides services such as:
  • Domestic Assistance
  • Social Support and Transport
  • Shopping Assistance
  • Meal Preparation and Meals on Wheels
  • Home Maintenance Including Lawn Care

These services help people maintain independence, safety and wellbeing while remaining at home and connected to their community.

Transition to Support at Home

The Australian Government is gradually reforming in-home aged care through the Support at Home program.

The Commonwealth Home Support Programme (CHSP) will continue to operate during this transition and is expected to move into the Support at Home program no earlier than 1 July 2027.

If you currently receive services through CHSP or previously received a Home Care Package, your services will continue during the transition period. Wenonah Community Care will support participants and families through any changes as they occur.

Funding levels and any participant contributions are assessed by Services Australia based on individual circumstances.
